SOMETIMES YOU JUST

NEED SOME HELP

Community Services

One of the most important things we have is our community. We strive to ensure that each individual we serve has the opportunity to become an active member of their community, building lasting relationships and gaining self-sufficiency through skills training, volunteer opportunities, social activities, and meaningful employment.

  • Interpersonal networking, individualized skill development, and personal assistance are the keys to our Community Services program. We provide individuals access to social clubs, recreational activities, volunteer opportunities, and community outings, all with the goal of building the skills they need to create their perfect life.

Through our community activities and resources, including our full Day Program, our dedicated staff ensures that each individual is given the opportunity to develop skills and build self-confidence in a safe and supportive environment.

Our staff provides the necessary help when needed, such as bathing or toileting support, all while encouraging growth and self-sufficiency.

From learning to do their own laundry to handling their own money to general public etiquette, individuals are supported daily in their efforts to learn and grow according to their unique situation.

Employment Services

We believe that real opportunities should be available to everyone. RISE provides in-depth career exploration, planning and training, each uniquely tailored to the individual. And for individuals who would benefit from it, we provide prevocational services to prepare them for their future career aspirations.

  • Setting real goals and achieving real results. That’s the goal of our employment services. Through our career planning service, individuals are given the opportunity to work with experienced job coaches who will help facilitate a career plan that is as unique as the individual themselves.

Using a combination of interest assessments, informational interviews, and job shadowing, job coaches help to determine the best path for each individual based on what they like and what types of jobs their skills are best suited for.

Job coaches are also given the chance to observe each individual in their work setting, creating a powerful opportunity for feedback and growth.

Career Planning is a 3 month program.

For those who aren’t quite ready to join the work force, we also offer the self-affirming opportunity to build general prevocational skills such as communication, following directions, problem-solving skills, workplace safety, and more.

Residential Services

We know that each individual deserves the right to live their unique perfect life. RISE helps make that a reality by providing the right supports to individuals in their own home or within a group home setting. No matter where we serve, our goal is to foster an environment of safety, growth, and self-sufficiency as each individual pursues their perfect life.

  • Home should be your perfect place. The place where you feel comfortable, safe and cared for. At RISE we believe that having a safe and healthy home is an essential part of the perfect life.

Depending on the needs of the individual, RISE offers residential supports through either Individualized Support Living (ISL) or in a Group Home setting.

Individuals who benefit from ISL are happy to be in their own place, but they can still use a little help. Our staff provides assistance and training with the general activities of living (cooking, cleaning, home maintenance) along with opportunities for outside community engagement.

For individuals who are better served in a Group Home setting, our dedicated staff offers them the opportunity to develop the same skills as those we serve through ISL (cooking, cleaning, home maintenance) along with same chance for community engagement, all while receiving the in-home oversight and health and safety monitoring that the individual may require as they seek their perfect life.

Vocational Services

RISE provides a Missouri Department of Secondary and Elementary Education (DESE) certified Sheltered Workshop where individuals are mentored and trained through a wide range of jobs, learning valuable skills with the goal of transitioning into community-based employment.

  • The RISE Vocational Center is where individuals can learn employment-related skills in a safe and nurturing environment. Our dedicated staff of experts work with each individual, training them across multiple employment areas, including: production, food service, retail, janitorial, woodworking, and groundskeeping.

DESE certified employment status us required.

Transportation Services

RISE recognizes that transportation is a vital part of our mission to support community inclusion, self-sufficiency, and individual choices. To better serve that mission, we maintain a fleet of vehicles to meet the unique needs of those we serve, each driven by a trained, dedicated professional. Our drivers not only care about the safety of their passengers, they are fully invested in the RISE mission of service.

  • We are a contracted provider for the Department of Mental Health and abide by all Missouri Department Of Transportation requirements regarding the transportation of individuals with disabilities.

Each driver undergoes a thorough background check, is trained according to exacting federal standards, and is continually monitored to ensure the safety and health of those we serve. This also includes regular CPR training, first aid, and instruction on proper lifting techniques.

All drivers are screened for alcohol and drug use.

Transportation funding is available through a Department of Mental Health waiver or the RISE Transportation fund.

Community Specialist

When you live with a disability, understanding and navigating services can be overwhelming. The RISE  Community Specialist helps people find the benefits and services they need to support a full life. They can help guide you through the entire process of identifying and applying for services and act as an advocate to ensure you are receiving the help you need.

  • What can I expect when meeting with a Community Specialist?

    Initial Interview

Our specialist will sit down and learn about the most important part of the process: you. By taking the time to ask the right questions we begin to build a plan tailored to you.

    Identify Benefits and Services

Once they know a little more about you, our specialist will help identify the services that you might benefit from. This may include transportation services, day programs, in-home support, job coaching, vocational training, and more.

    Take Action

Once a plan is in place, our specialist will walk you through the process of applying for the various services. This may include working with the Missouri Department of Mental Health, the Johnson County Board of Services, local school programs, the Medicaid program, and others.

    Continued Support

Our specialist will continue to support you after the initial process. They can help advocate for you to ensure that you are receiving the help you need.
