Individualized Supported Living

The goal of Individualized Supported Living (ISL) is to provide residential supports to a resident in their own home. Residential supports are designed to assist and teach home maintenance skills such as:

  • Activities of daily living (cooking, cleaning, home maintenance)
  • Community engagement

RISE does not have ownership over ISLs and does not have authority to move residents out of their homes. RISE will support financial and bill management for ISLs. Transportation and Health Services are included alongside residential services.

Group Home

The goal of Group Home is to provide residential setting that promotes skill development while delivering oversight and monitoring for health and safety of the resident. Residential supports are designed to assist and teach home maintenance skills such as:

  • Activities of daily living (cooking, cleaning, home maintenance)
  • Community engagement

The Group Home is owned and operated by RISE. RISE is responsible for the home and billing maintenance of the home. Transportation and Health Services are included alongside residential services.

Respite

The goal of Respite is to provide planned relief to the caregiver(s) of the service participant. Respite supports include:

  • Skills development
  • Specialized care to support the individual’s unique needs
  • General oversight and monitoring for health and safety

Respite can be provided in the participant’s home or out of home at RISE’s group home.

Respite is a temporary support, not a permanent placement.
